Survey Completion Notes
Conditions of Entry:
1. Information on prizes and how to enter form part of these conditions of entry.
2. Entry is free and open to all households of Australia 14 yrs of age and over.
Employees, directors, managers, licensees and contractors of the Promoter, its related companies
and their agencies (and families of each of these) associated with the survey are not eligible to enter.
Retailers of and suppliers to the promoter are not eligible to enter
3. One entry per household
4. To enter the promotion, entrants must provide a valid name and address and complete the survey.
5. Free maths assessment is offered with every entry to families with school age children
that are selected to view the Maths Wiz programs.
6. There are two competitions being conducted. One prize will be awarded at the end of each competition.
7. The first competition period was from 1 July 2008 to 12 December 2008.
The prize for this competition was drawn at 5.00pm on 18 December 2008 at Level 1, 479 Warrigal Rd Moorabbin VIC 3189.
8. The second competition period is from 13 December 2008 to 30 June 2009.
The prize for this competition will be drawn at 5.00pm on 6 July 2009 at Level 1, 479 Warrigal Rd Moorabbin VIC 3189.
9. The prize awarded to the winner at the end of each competition period consists of, a notebook PC
with Intel Single/Dual Core or equivalent with a Maths Wiz program loaded.
The Maths program covers 1 level of the winner’s choice.
10. The winner of each competition period will be notified in "The Australian" newspaper (public notices section)
on 21 December 2007 and 12 July 2008 respectively.
11. The prize has a r.r.p. up to $2,490.00 for each draw (total prize value of $4,980.00).
12. The prize is not redeemable for cash or other merchandise.
13. If required, unclaimed prize draws will be held 2 April 2008 and 8 October 2008 respectively.

15. Kinetic Education ensures that your information is protected by the Privacy Act 1988 (Cth) at all times.
16.The information you provide on this leaflet may be used by Kinetic Education and its distributors
to contact you for the purposes of informing you on the company's products and services.

18. The Promoter, its contractors, employees and agents, shall not be liable for any
claims, losses, damages, injuries, costs and expenses suffered or sustained or incurred
(including but not limited to indirect or consequential loss), arising out of or in
any way connected with the competition and/or its prizes except for liability that
cannot be excluded by law. The Promoter accepts no responsibility for any tax
implications that may arise from the prize winnings. Independent financial advice should be sought.
19. Entry into the competition shall be deemed acceptance of these conditions of entry and is deemed
approval to use the winners’ name(s) and photograph(s) for publicity purposes.
The Promoter may, at its discretion refuse to award any prize to any entrant who fails to
comply with these conditions of entry. The Promoter’s decision is final and no correspondence
will be entered into. All surveys received become the property of the Promoter.
20. The Promoter assumes no responsibility for any error, omission, interruption, deletion, defect,
delay in operation or transmission, communications line failure, theft or destruction or
unauthorised access to, or alteration of the competition. The Promoter is not responsible for any
problems or technical malfunction of any telephone, network or lines, computer online systems,
servers, or providers, computer equipment, software, technical problems or traffic congestions
on the Internet or at any web site, or any combination thereof, including any injury or
damage to entrant’s or referral’s or any other person’s computer related to or resulting from
participation in or downloading any materials in this competition.
